1. Schedule your appointment

Call 1-855-485-4673 to get an appointment with an HonorHealth Cancer Care provider – within 48 hours, if desired. Please note that you'll need a referral from your primary care physician, or specialist.

2. Complete your new patient forms

Please download and complete all of your new patient forms and bring them with you to your first appointment. These forms are meant for patients in medical oncology, hematology and GYN oncology. 

3. Prepare for your visit 

In addition to completing the forms, please review the following documents prior to your visit. 

You’ll also need to bring the following with you on your first visit:

  • Photo ID 
  • Insurance card 
  • List of all medications 

4. Meet with your care team

Your oncologist will review your diagnosis and imaging, laboratory and pathology results to discuss your customized treatment options. You should know that your doctor will collaborate with as many as 35 specialists through multi-disciplinary planning group meetings. There, they'll discuss and strategize the best treatment approach for you.

These detailed discussions also enlist the expertise of genetic counselors, reproductive medicine experts and clinical research staff. The goal? Ensuring that you receive a customized, comprehensive treatment plan for your specific cancer type, stage and unique life/family situation.

5. Meet with a cancer care navigator 

Your initial appointment will be followed by a meeting with an HonorHealth cancer care navigator, who will assess your needs, review your concerns and recommend complementary therapies and support services that you might find helpful. Such services may include nutrition counseling and support, assistance from a certified cancer social worker, massage therapy, acupuncture, yoga, tai chi and more.

Throughout your treatment journey, your cancer care navigator will serve as a liaison between you and your multidisciplinary care team. Your navigator will help guide you through your individual cancer treatment plan from start to finish, helping with:

  • Scheduling follow-up appointments
  • Managing referrals
  • Offering assistance dealing with the financial and insurance aspects of treatment
  • Keeping you informed along the way
