HonorHealth Outpatient Therapy - Deer Valley is located on 27th Avenue across the street from the Deer Valley Hospital. It is just south of State Route 101. From the Loop 101, take the exit for 27th Avenue and the Deer Valley Hospital.  Travel south on 27th Ave., then turn west (right) after the first light.Please note that we have two separate office suites:Upon entering the parking lot, the building to the south is 19636 N 27th Ave Ste LL2, and the office is located in the basement.The second office is in the building to the north, 19646 N 27th Ave, Suite 405.Please confirm with our team which building your appointment(s) will be in to avoid confusion.
