Overview
The HonorHealth Pulmonary Hypertension Program offers a multi-disciplinary team that focuses on early detection, comprehensive evaluation, and aggressive treatment of pulmonary arterial hypertension. The program is patient focused with care plans that evolve throughout the evaluation and management of this disease state, embracing each individual’s uniqueness. Ongoing education and support enhances the patient experience. Collaboration with the patient’s physicians and care providers is a respected value that can be observed at any stage of care.
