We’re excited to share that HonorHealth has officially welcomed 11 new primary care locations into our family through an acquisition of Evernorth Care Group. That means you now have even more options across the Valley, and it’s easier to get the care you need, when and where you need it.

All locations acquired by HonorHealth are now open.

 

Scheduling an appointment with HonorHealth

Starting Monday, Jan. 12, you can schedule a primary care visit or video visit by calling  623-580-5800.

Please note: Our scheduling team is currently experiencing a high volume of calls. If you prefer not to wait on the phone, you’re welcome to use our call back feature, when offered, to save your place in line and receive a return call from an HonorHealth team member.

Most physicians and advanced practice providers have transitioned to a nearby HonorHealth Primary Care location. See our Physician Directory to find where your provider is now practicing.

If your previous physician did not join HonorHealth, we're happy to help you find a new primary care provider in our network. Call 623-580-5800 for assistance.

 

Locations not acquired by HonorHealth

We did not acquire Arrowhead Ranch Center, Chandler Health Center, Paradise Valley Health Center, Paseo Health Center, Scottsdale Norte Health Center, Sun City Health Center or Westridge Health Center. These locations are closed.

To continue receiving appointment reminders and messages from your care team, please create an HonorHealth MyChart account. It’s our free, easy-to-use app and website that gives you quick, convenient access to your health information. With MyChart you can schedule appointments, see lab results and more.

Evernorth Care Group securely transferred all patient prescriptions to Walgreens in mid-December 2025.  For questions about your prescriptions, please call 800-WALGREENS (800-925-4733).

You are welcome to use any other pharmacy you prefer. To do so, simply contact your new, preferred pharmacy directly, and they will handle the entire transfer process for you.

Please note: Some medications, including controlled substances or those needing a prior authorization, require a doctor’s visit before they can be refilled. At HonorHealth, we offer several convenient options, including video visits, to help you get your medication refilled.

You will not need to request your medical records from Evernorth Care Group to be seen by your physician who transitioned to HonorHealth. General medical information will be securely transferred as part of the transition. Please connect with your physician to determine if a medical record transfer is necessary.

You can continue to access your historical medical records through your Evernorth Care Group MyChart account through March 31. After that time, please call 480-345-5171 or email evernorthcaremedicalrecords@evernorth.com to request copies of your records.
