Frequently asked questions
Does my insurance cover the membership fee?
A: No. Membership fees cover your premium access to Dr. Levine and are not billed to insurance. All clinical services, including imaging and diagnostic testing, will be billed through your insurance.
Are payment plans available?
A: No. Payment plans or monthly installments are not available at this time. Your annual membership fee or one-time consultation fee will be due when you enroll.
What forms of payment does the program accept?
A: Credit cards, debit cards and checks are accepted for all membership and consultation fees. These fees may be paid by phone.
Can I use my HSA or FSA to pay the membership fee?
A: No. Concierge membership fees are not classified as a qualified medical expense and cannot be paid using HSA or FSA funds. Clinical services billed through your insurance — including standard visits, imaging and diagnostic testing — may be eligible for HSA or FSA reimbursement depending on your plan. We recommend confirming with your plan administrator.
What is the difference between the three-month membership package and the single consult?
A: Our three-month membership package ($2,500) is a comprehensive evaluation over a period of three months that includes advanced diagnostic testing, risk factor analysis, care plan and follow-up visits. It’s designed for patients seeking a thorough neurological workup or exploring annual membership. The single consult ($1,000) is a focused, one-time expert review for patients with an existing diagnosis who want expert guidance before committing to a prognosis or treatment plan.
What happens after the three-month membership package?
A: If you choose to enroll in annual membership within 30 days of your last consultation, your $2,500 payment will be credited toward your first-year membership fee. If you're not ready to commit, you'll still have a comprehensive picture of your neurological health and a personalized care plan.
If I purchase the second opinion package, can I keep my current neurologist?
A: Yes. The second opinion consultation is designed for patients who want expert input without switching providers. You may enroll as a full member afterward or keep your current provider — that decision is entirely yours.
Do I need to be a current HonorHealth patient to join?
A: No. HonorHealth Concierge Neurology is open to anyone, regardless of where you currently receive care.
How many members does the program accept?
A: To protect your expanded access, membership is capped at 275 — compared to thousands of patients with traditional neurological care.
Where is HonorHealth Concierge Neurology located?
A: We are located at the Bob Bové Neuroscience Institute at HonorHealth, 7242 E. Osborn Road, Suite 500, Scottsdale, AZ 85251. We serve patients throughout the Phoenix metro area, including Scottsdale, Tempe, Chandler, Gilbert, Glendale and Peoria.
