Lifestyle Medicine Primary Care
Feel better today. Build a stronger tomorrow.
Ready to feel your best every day, not just at your checkup?
Most of your health happens between visits, not in the exam room. What if your primary care experience included additional support to help you feel your best every day?
HonorHealth Lifestyle Medicine Primary Care is an optional membership program that complements your medical care with guidance focused on healthy habits and long-term well-being. It doesn’t replace traditional primary care. It builds on it.
You'll work with a team trained in lifestyle medicine to build sustainable routines around nutrition, physical activity, sleep, stress management and other factors that influence your health.
The goal? More energy, feeling your best and a lower risk of chronic illness over time.

Frequently asked questions
About the program
Q: What is HonorHealth Lifestyle Medicine Primary Care?
A: HonorHealth Lifestyle Medicine Primary Care is a membership-based program that complements your primary care with personalized lifestyle coaching and access to a registered dietitian. It’s designed around the six evidence-based pillars of lifestyle medicine — nutrition, physical activity, sleep, stress management, social connection, and avoiding risky substances — to help you make sustainable changes, feel your best and lower your risk of chronic disease over time.
Q: How is this different from traditional primary care?
A: Traditional primary care is focused on diagnosing and treating illness. This program adds a proactive, habit-focused layer — with a smaller patient panel, a registered dietitian, and a personalized plan built around the six pillars of lifestyle medicine. The goal is to help you address everyday factors that influence your health so you can prevent or better manage chronic disease and enjoy a higher quality of life.
Q: Is membership required if I want to receive ongoing care from Dr. Cupic?
A: Integrative Medicine is a specialist consultation that focuses on complementary therapies like supplements, herbal medicine, acupuncture, and mind-body approaches. It is not a primary care practice. Lifestyle Medicine Primary Care takes the place of your primary care physician. If you already see one of our Integrative Medicine providers, this program gives you a primary care doctor who is philosophically aligned with that care, so your providers are always working in the same direction.
Q: If I’m currently a patient of Dr. Cupic and choose not to become a member, can I switch to another HonorHealth primary care provider?
A: Lifestyle Medicine Primary Care is designed for patients who want a proactive, lifestyle-focused approach to their health. Concierge Medicine offers enhanced access to your physician after-hours, on weekends and on holidays as well as same-day appointments and a limited number of patients. Lifestyle Medicine Primary Care is focused on the lifestyle and preventive care coaching program, rather than enhanced physician access. The membership fee reflects that difference — just $85 per month, compared to Concierge Medicine pricing.
Q: What happens at my first visit?
A: Your first visit is a 60-minute appointment with Dr. Cupic, where she will get to know your health history, your goals and where you’re at. Together, you’ll identify opportunities to build healthier habits, and your care team will begin creating a personalized plan to support your long-term health.
Q: Are telehealth visits available?
A: Yes, telehealth is available for appropriate visit types. Please call us to confirm whether your specific visit can be conducted via telehealth.
Billing and membership
Q: Is there a waitlist for Lifestyle Medicine Primary Care?
A: Once a physician reaches capacity, you can put your name on a waitlist. As openings become available, we will contact those on the list (in order). Additional providers and locations may be added in the future, based on demand.
Q: Is this a monthly or annual membership, and what is the membership fee?
A: This is an annual membership. You can choose to be billed monthly at $85 per month or save 10% by paying your annual membership fee up front at $918 per year.
Q: How do I become a member?
A: You can enroll by calling 480-587-6930 or visiting us at 8405 N. Pima Center Pkwy., Suite 203, Scottsdale, AZ 85258. Our team will guide you through enrollment and answer any questions you may have.
Q: How does payment work?
A: At signup, you can choose to be billed monthly or annually. At the end of each enrollment year, your membership will auto-renew for an additional year unless you call and cancel 30 days prior.
Q: Will insurance pay for the membership fee?
A: No, the annual membership fee is not covered by insurance. However, traditional primary care services such as office visits and lab tests will be billed to your insurance as usual.
Q: What is not included with membership?
A: The membership does not cover clinical medical services. Office visits, lab work, imaging, prescriptions, specialist visits, and all other medically necessary services are billed to your health insurance separately. Copays, deductibles, and coinsurance may apply depending on your plan.
Q: Does the program accept Medicare, Medicaid and other insurance plans?
A: Lifestyle Medicine Primary Care membership fees are not covered by Medicare, Medicaid or other insurance. However, the program accepts major insurance plans for office visits and other clinical services. See a list of contracted plans.
Q: Will I still have copays and deductibles?
A: Yes. Copays, deductibles, and coinsurance apply to clinical services as determined by your health insurance plan. The membership fee is separate and does not affect how your insurance processes claims for clinical services.
Q: Is Lifestyle Medicine Primary Care a supplemental insurance policy?
A: No. Lifestyle Medicine Primary Care is not insurance. HonorHealth will continue to bill your insurance for clinical services such as office visits and diagnostic tests, except for the three dietitian visits included with your membership fee. If specialty care or hospital services are needed, we will bill your insurance as usual. If you don’t have insurance, you can self-pay for primary care services.
Q: Is the membership fee eligible for FSA or HSA reimbursement?
A: No. The membership fee is not eligible for FSA or HSA reimbursement. Clinical services billed to your insurance remain separate and may qualify depending on your plan — please check with your benefits administrator.
Q: What happens if I need more than three dietitian visits in a year?
A: Three registered dietitian visits are included in your annual membership. Additional visits beyond that would be available on a self-pay basis. Your care team can help you determine the best approach based on your needs.
Q: What if a medical issue comes up during my lifestyle evaluation visit?
A: The annual lifestyle evaluation is a dedicated coaching visit focused on your health habits and goals. If you require medical attention, a separate appointment will be scheduled and billed to your insurance as normal.
About your physician
Q: If I join Lifestyle Medicine Primary Care, would Dr. Cupic become my primary care provider?
A: Yes. Dr. Cupic is a primary care physician — she handles the full scope of your primary care needs, including managing chronic conditions, ordering labs, prescribing medications and referring to specialists when needed. The membership adds lifestyle coaching support and access to a registered dietitian on top of that.
Q: Will Dr. Cupic still be seeing patients at her current location?
A: Dr. Cupic is currently seeing patients at an additional HonorHealth location as she builds the Lifestyle Medicine Primary Care program at the Pima Center. Over time, her practice will transition fully to this model. Current patients will be notified directly and well in advance of any changes to their care. As always, continuity of care is our priority.
Q: Is a membership required if I want to receive ongoing care from Dr. Cupic?
A: Yes. However, there will be a transition period for her current patients at HonorHealth Medical Group - Carefree Highway - Primary Care.
Q: If I’m currently a patient of Dr. Cupic and choose not to become a member, can I switch to another HonorHealth primary care provider?
A: Yes. At HonorHealth, we offer a variety of primary care options to meet your needs. This membership program is one way to access care, and traditional primary care is always available. Need help finding a new provider? Call 623-580-5800 and we’d be happy to help.
