Continuity clinic
Our Family Medicine Residency Program provides continuity clinic experiences at two distinct sites: HonorHealth Medical Group Heuser and River People Health Center. Together, these clinics offer residents an outstanding continuity experience where they provide full‑spectrum primary care to patients from diverse backgrounds, with training in tribal health, Hepatitis C management, LGBTQ+ care, trauma‑informed care and more.
Our continuity clinic sites are committed to the delivery of high-quality medical care that is patient-centered, safe, equitable, evidence-based and cost-conscious. We partner with our patients and community to optimize health across the lifespan by providing compassionate and comprehensive care while educating the next generation of family medicine physicians.
HonorHealth Medical Group - Heuser - Primary Care
Located just next to HonorHealth Scottsdale Osborn Medical Center, HonorHealth Medical Group - Heuser - Primary Care is a 12,000-square-foot practice with 18 exam rooms, two procedure rooms and a laboratory. Patients can also access radiology services within the clinic's office complex.
River People Health Center
River People Health Center is located just 15 minutes southeast of the HonorHealth Scottsdale Osborn Medical campus and provides comprehensive, culturally respectful care to members of the Salt River Pima‑Maricopa Indian Community. River People Health Center offers a wide range of services delivered within a model that integrates physical, emotional, mental, and spiritual wellness and honors local Native cultural traditions.
We provide a setting where each resident physician develops a practice that provides comprehensive primary care to a continuity panel of patients. Each resident belongs to a patient care team that emphasizes and facilitates continuity and team-based care.
- PGY‑1: 1–2 half‑days per week in continuity clinic
- PGY‑2: 2–3 half‑days per week in continuity clinic
- PGY‑3: 3–4 half‑days per week in continuity clinic
Specific areas of focus that help to prepare residents for real-world practice after graduation include:
- Patient-centered, interprofessional team-based model of care
- Coordination of care and chronic disease management, including patient panel management and population health
- Community resources to address social determinants of health and healthcare disparities
- Integrated behavioral health, nutrition and clinical pharmacy
- Billing and coding education
- Excellent patient and office workflow design
- High levels of physician and patient centered continuity
- Personnel issues including conflict management
- Telemedicine and other ways to maximize point of care technology
- Longitudinal patient experience training
